Just to add to LaVonna's excellent information:
If you can put your hands on a copy, The GUI Style Guide has a section on
defining hazard messages and icons. It summarizes the Kemnitz article, an
article by d'Albenas in Intercom (Oct. 1992, pp. 3-5), and the ANSI standards,
and lists the ANSI standards on warnings. It also includes the ANSI address and
phone number (212/642-4900).
Added value: a section on defining warning sounds.
