A colleague of mine has solved the mystery. It seems that some of the
Figure Caption tags were somehow being confused with the ChapNum tag in
the chapter before the bad one, so Frame was counting certain screen
shots as chapter numbers. We fixed it by applying the format of a "good"
chapter to the chapter before the "bad" one. Voila! Thanks to everyone
who tried to figure this one out...
