First of all, thanks to all who replies to my query re
headers/footers.separators.
Everyone noted the necessity of separating the header/footer information
from the body text and the majority favoured the use of a line separator
(of varying degees of thickness!). Those who didn't like the line separator
used white space to create the required break (some used both).
As to content there was again a general consensus that there should be no
header on the first page of a chapter/section (info should be obvious on
the title page) and that the most "important" information should be on the
outside edge of the page. The page, as a whole, should look balanced -
line in header then ditto in footer, left/right info in the header then
ditto in the footer. Minimum use of fonts (suggested using heading font,
but smaller, for header/footer. Specific suggestions received are given
below. Some give split beteen left/right while others simple suggest
information which should be displayed.

Righthand page (RH) : Left - manual title Right - chapter/section
title
Lefthand page (LH) : Reverse of RH page
LH : Left - manual name Right - nothing
RH : Left - nothing Right - Chap/sect number & title
Look once info in the header - book/volume/section title

Footer:
RH: Left - version/release date Centre -
copyright/classification/comany name Right - page number
LH: reverse of RH
RH: Left - Version no./release date Right - Page no. (3-1 etc)
