First, set up the book file. Set Create Hypertext links on the TOC file.
Then regenerate/update. You will have a hyperlinked TOC (click a title, go
to the page).
Make sure that you have _no_ margins set in your Acrobat Distiller/print
options. You change that option by the Printer Setup button when you go to
print the file to ps or pdf.
