Bad grammar is what bad grammarians practice.
Bad English grammar is any view of the structure of the English language
which cannot explain constructions that actual English speakers use to
communicate effectively.
Since the phrase "Click Apply, then OK" would seem to communicate
effectively, bad grammar would be any grammatical system that cannot account
for it.
