time, how much does it cost to fix errors? Nothing. How long does correction
take? No time. The key, then, is to do things right the first time.

In terms of the Capability Maturity Model (CMM), the vast majority of
software companies are at level 0 to 1. Developers in such environments are
totally incapable of "doing it right the first time". Tech writers
(downstream of design) can not hope to do better (unless one has a very
loose interpretation of what "doing it right" means).
