Both.
When it was the information structure, I probably could have
resisted the urge to tinker and got away with it. However, I do a
lot of first passes through documentation, so my understanding of
the material is usually increasing as I write. And, even if I'm
completely on top of a subject, I constantly have inspirations for
new ways to present material (some of which work out, some of which
doesn't). Also, since I have a strong interest in typography, I also
like to experiment with layout.
All of which means that there's only so much planning that I can do.
I know, for example, that I like two levels of bullets and
corresponding indents. However, I don't always know if I need both
alphabetical and numeric lists, for example, or whether I'll want to
include so many charts that I want captions and caption headings. In
many cases, I suppose, I could put everything in just in case, but I
like to keep my paragraph tags or elements to a minimum.
