well, somebody has already said this, I'm sure, but....
as William Faulkner once said, "there is no was, only is"
and, for online help, there are no sections or chapters, only topics.
OLH tends to present a reference as <hyperlink>topicname</hyperlink> and be done
with it.
but printed matter at least needs a topic name and page number.
so the biggest annoyance, imho, would be to make yr ss tool realize that it can
strip "on page 1999" phrases from the original text, while yr print manuals
would keep the page ref.
and then start addressing those nasty Heads that simply say "the following
topics address <<stuff>>" and then launch into the next lower head level.
