OTOH, a couple of places that I've been have put manual title, chapter title,
section title, and chapter-page numbering in the headers and footers. In those
cases, the "chapter" bit could be a useful way to distinguish the various
titles.

Also, I'd tend to use "chapter" if the actual chapter heading used "Chapter".
Its a trivial bit of information, but why present it 2 different ways?
