RE: cross-reference markers and online help

Edit the cross-reference definition. Get rid of <$paratext> and just use the
word "manual" (no quotes). Then use that cross-reference definition when
linking to the manual's title. You should just get the word "manual" in your
output... hyperlinked, of course.
