Take a close look at the initial T in the Word heading. Is it a
different font from the rest of the line? Bold, while the rest of the
line is not bold? Have Shadow turned on (check the Format > Font dialog
with just the single character selected)? Also hit Alt+F9 in the Word
doc to see if you have any hidden codes just after the initial. Finally,
click the paragraph mark button to show invisible characters. Pay
attention to extra line spaces above headings (they should be in a style
that does not have a TOC attribute, such as Normal). Also pay attention
to headings that you break into two lines. Be sure you used Shift+Enter
rather than a hard return (paragraph).

I'm using section breaks/next page throughout the document. Where this
problem occurs, the paragraph mark just prior to the section break has
either a Heading1 or Heading2 style. I changed a few of these to body and
rebuilt the PDF and the problem went away.
The paragraph marker looks like a body paragraph marker (not large like a
Heading1 or Heading2 marker)so I assumed (hate that word) that it was a body
style. I even did a search and replace for a paragraph marker with a Header1
or Header2 style and none of these were found. Thanks to Dick for suggesting
I look a bit closer. Problem solved.
