You get a gray box because the EPS fudged by FM upon
import of a PDF may not have created a preview for the
EPS. Create an EPS either from Illustrator or from
Acrobat and import that. make sure you create an EPS
with a preview image, otherwise you'll still get the
gray box in FM.
