I've used Sonar Bookends Activate from Virginia Systems for about two years now.
This one-trick pony couldn't be easier to use: Open the PDF, select the tool, enter the physical page number where the index starts and the number of frontmatter pages (i, ii, ...), and press Enter.
Converting 30 large indexes should take you less than half an hour. You'll probably spend more time opening and saving the PDF files than Acrivate will spend hyperlinking the page numbers. (Maybe that's the manual work you're referring to...)
