Twice. Once, I was asked to document how to use a paper clip. It took so
long for them to call me for a second interview that I already had another
job when they did.
Another one was pretty interesting. I was given a test with a variety of
"personality" type questions. After completing the test, the HR type told
me my score meant that I was "executive material." For some reason, I never
got called back for a second interview.
