Check the TOC Reference Page (View->Reference Pages) and ensure that the
marker for the tag being used for the Glossarty and Index have a <$pagenum>
variable.

Have you verified the numbering properties (Format>Document>Numbering)of
the glossary and index? Check the Chapter, Page, and Paragraph tabs to make
sure the right options are selected. Do this from within each chapter, but
also from the book, by right-clicking each chapter.
