My design has chapter, appendix, and index headings that
look roughly like...

Appendix C
System calculations
Z
Index
The autonumber is flush left and 12 pt Arial Bold. The
chapter title is 16 or 18 pt Arial Bold, and is right flush.
I could change the "Z" to be white or hidden or some such,
but it doesn't seem to cause too much trouble. And it
carries into the TOC perfectly.
The corresponding page numbers are 1.1, C.1, and Z.1.

Is this just a file-naming convention, as for your front
matter, or does
your index actually start with a heading that says "Appendix
Z. Index"?
My impression was that the original question had to do with
customer-visible
headings and page numbers, not file naming conventions.
