Which comes first: content or design?

For technical documentation? Content, always.

Having layout niceties like no very short lines is icing on the cake as far as technical documentation is concerned. Some of this icing comes cheap, like no single lines of a paragraph left alone on a page (widows or orphans), which you can specify in the paragraph format. Since the effort is minimal, you usually go ahead and do it.

Icing that requires a lot of manual work, like making sure there are no very short lines, usually isn't worth the bother and money it costs.

If you cannot agree with your graphic designer, make it a management issue: Estimate the time it would cost you to fix this issue, estimate the resulting release delay for the documentation and the cost, and let your manager decide.
