Have you tried both Adobe Distiller and PDFWriter?
Have you adjusted the DPI settings in the Distiller Job Options?
Have you tried MyMorph? I tried it on a few of my files, and it created
significantly smaller files than Acrobat (but without linked bookmarks -- I
couldn't tell from your letter if that matters in your case). MyMorph is
free, and can be found at http://docmorph.nlm.nih.gov/docmorph/default.htm.
