I don't know about anyone else, but I use Headings for heading text
and certainly don't associate heading levels staticly with content
that may appear in various places in the hierarchy of a book. That is,
I don't follow Heading 2 with Heading 5 for the sake of formatting.


Fair enough, but missing the point. If you designate a style (call it ListTitle instead of Heading5) that provides a visual clue that a list follows, punctuation becomes superfluous. That style can occur within any level of the hierarchy; it isn't part of the outline; it's just a typographic device.

Personally I don't use more than three levels of heading (as in outline levels that show up in the TOC) if I can avoid it.
