I've replied to Elizabeth nut this may link to the Communication
Research of Australia may well be of more general interest.

They've done a lot of work on form design (among other things) - I
remember reading a paper by them about re-designing a tax form for the
Australian government that was _longer_ than the original but, because
of the design, was quicker to input and saved $100,000s for the
government.

This was a few years ago now and all their articles etc were free to
download. They were in the process of re-jigging their site so this may
not be the case now; I think they were moving towards providing samples
and then the rest to those that 'paid'.
