Don't use the SaveAs function. Set your default printer to the Acrobat
Distiller print driver instance. Open Frame and print to a file. Then
Distill the file.

Help! Frustrated, pullling out my hair, deadline 2 hours away.
Situation: Generate PDF from FM book using Save Book as. When PDF opens,
1st page of 2nd appendix displays; bookmarks start at chapter 4.

Acrobat Professional 6.0
PDF settings: Job options: high quality
Open on page i (cover), a zoom Fit Page
no registration marks
Range: all
Bookmarks: generate PDF bookmarks checked, expanded through all
Tags: none
Links: create checked
I've used this same set up for 2 other books in the doc set which was
highly successful and painless, yet this one is giving me more grey hair
than I had this morning. I've regenerated the book 5 times. Backed out
of FM and Acrobat, shut down the system and rebooted, and all those
other good things.
