As almost always, it depends... :-)
Generally, I would use font size/emphasis/positioning to visually
differentiate between heading - the degree of differentiation needed
will vary, you could get away with minimal differentiation if your
headings are numbered.
Space before I'd have larger than space after, indicating that the
heading is linked to the following text. The amount of space left
depends on a number of things - the font used, aesthetics, etc.
