upper management, better at a lower level, and if possible delgated to

people. Just include the departments that are vitally concerned,
sales/marketing, engineering (R&D), manufacturing, whichever departments
are concerned. All others can be consulted after the initial triage, or as
research prior to triage.
Should require no more than three signatures to get an enhancement moving.
It should then go to the managment group for consideration as a project or
inclusion in a project.
At this stage of a companies life, it should be kept as simple as
possible, or the process will paralyze you, or get dumped because it isn't
workable.
