Hey, Mike. My experience is congruent with your recollection: you
have to do it in Acrobat.
Like you, I think that there *ought* to be an easy way to code up a
PDF form *outside* of Acrobat and then send it off to Distiller for
processing. T'aint so. The actual placement of form fields, etc. has
to be done within Acrobat.
