We all learn.

I just had some materials  that I looked at that discussed what might be 
called extensions.

The  material was dated in 1978, in the "Stored Program Control, Software  
Documentation Standards by AT&T, PUB 53300, copyright 1982.

It was  not my intent to state that someone other than MS set a 'standard'. 

With  all of the 'turmoil' in the early 80's concerning MS/DOS, CP/M. and 
others, many  players were involved.  IBM, NCR, Tandy (TRS), and others were all 
probably  working together to define the language, the structures, and the 
environments  that eventually became MS/DOS, PC/DOS, MS Windows, and the ilk.

The  information that you provided in your email is correct in that it 
'formalized',  and documented in one place what people had been doing for years. MS 
Windows  justified the 'ad hoc' way people were doing things.
