If the book is made available as a PDF file with bookmarks:

Regardless of the location of the Index in the publication, the Index bookmark can be placed in the top area of the bookmark list. This is especially helpful When there are many bookmarks and the entire list of bookmark cannot be displayed without scrolling, even when all top items are collapsed.
