A few years back, Jack Lyon did an excellent series of articles on Word
Indexing on his Editorium web site. He also has a plug-in known as
DEXter, http://www.editorium.com/dexter.htm, which you can try free for
45 days. I suspect the $199 investment to keep it could be well worth
it, in terms of time saved. I have no affiliation with Jack or The
Editorium, just appreciate the site and receive Jack's free newsletter.
If you want to read about technique but don't necessarily want the
plug-in, see the articles at http://www.editorium.com/euindex.htm.
Scroll down to Indexing. Topics go into more depth than the titles may
imply. They include:

Subject: Looking for a Word Indexing Plug-in
Hi All,
(Cross-posted to WORD-PC and TECHWR-L)
Can anyone recommend a Word plug-in that can create index files with
hyper links (similar to Word's TOC feature)?
I plan to turn these word files into PDF and would like to have
clickable index entries. The indexes are many pages long, so I'm looking
for an automated solution.
