How about,

"Attention: Complaint Department"

"Dear Program Director:"

"RE: Order #999999"

Or you can call the company and find out the name of the correct person to
address. I think the main interest here is to show that you are directing
your letter to a specific group or person and to avoid using vague
references, like "sir or madam" or "to whom it may concern."
