Daniel, have you you checked to make sure that you have Bookmarks
Expanded through Level - All on the Bookmarks tab in the PDF Setup
dialogue? That is accessible through FM's Format - Document - PDF Setup.
That may be all you need to get your bookmarks back.

Using Adobe PDF Printer is also bad news: You lose all of your
bookmarks. See my previous post on this, using PDF Creator as the
printer. Bookmarks ARE preserved.
Hmmm. Maybe Adobe should take a look at the PDF Creator source code
(FOSS, GNU license) to see how to do it right!

Did you try printing to the Adobe PDF printer instead of using Save As
PDF? It's long-standing advice in the FM world that if Save As PDF is
causing problems, try setting your printer to Adobe PDF and printing to
it. If it still doesn't work, set your printer to Adobe PDF, print to a
Postscript file (extension .ps), and then open that file with Distiller.
