In following the general principle of "write to your audience," it
sounds like the audience is stuck in the era of ancient pedantry (e.g.,
it's 2012 and they're *now* computerizing a *paper* form), so terms that
would be comfortable and familar to the audience is most appropriate.
That is a good point. Hopefully, the audience is not Amish...
