I had something similar happen once. It was happening to only one of several
books I was maintaining which were all based on the same template. I tried
everything I could think of to fix it with no luck. Somebody suggested
creating a new book file and generating from that. It took only a few
minutes and worked like a charm.
I can't answer why. We never figured that part out.
