Workaround: Decompile the CHM (or just start with the uncompiled directory). Put a copy of the directory in your Web root directory so you can access it with a URL. Open Acrobat. File > Open Web Page. Navigate to whatever you want to be the first page of the PDF. Check the Download Entire Site box. Click OK.
