Here's one to get you started:

(I whipped this up just now, in Gnumeric, so apologies if there are
formatting glitches in Excel. It *is* saved in Excel 2k format.
The math should be right.)

At top-left is a lookup table for "Mon", "Tue", etc. Could be moved.

Enter the start date for the whole sheet in F3, hourly rate in F4.
Enter before- and after-lunch start and end times in 24-hour format
("08:00", "17:00") or use AM/PM notation.

To make more weeks, copy last week's cell range and paste a new one
at the end, zero-ing the time values. No need to enter new dates:
everything continues from the root start date, and the cell
references are literal where they need to be.
