As to your q about 'all rights reserved': It's probably some kind of
sentence--maybe it's in incomplete sentence but anyway I am most comfortable
in styling it like a sentence (All rights reserved.). I've always just
assumed that it is an abbreviated form of 'All rights are reserved.'

Only the Name of Company, (not including LLC) is registered because business
structure (LLC, Inc., etc.) has changed several times over the years.
