Two things to check:
1. Are any of the tables in text boxes? If they are, they won't show
up in the cross-reference item list. Move the tables to inline or
convert the text boxes to frames.
2. Do you have change tracking on? If any of the tables or their
captions are being change-tracked, they won't show up in the list until
you accept their changes.
