If all changes have been accepted, try turning tracking off and then trying
the cross referencing again.
Another thing you can try is deleting the caption from one of the items
that are missing, copy/pasting in the caption from one that is working and
then renaming it. I don't know exactly why this works, but sometimes it
does.
